Below is a video walkthrough, but here's a cheat sheet on how to use the tool:

1. Extract the files to a folder.
2. Make sure you have .NET 4 installed.
3. Plug in your device, making sure you're in USB debugging mode (go into Application Settings to double check).
4. Run the AndroidCentral-SWM.EXE and browse for the .apk file that you want to install. Then hit the Go button. Bam!
